Our specially designed telephone- and Web-based support system enables you to take an active role in your ongoing care. Simply use your telephone or computer with Internet access to respond daily to automated questions about how you are feeling.
This information is securely reported to a heart failure care coordinator and your physician. An Inova HeartLink care manager reviews your information and contacts you or your physician if necessary to adjust medications or your treatment plan. These daily results help keep your care team well informed about your current health status, how your treatment plan is working, and how your medications and eating habits are affecting your condition.
